Starting Soolantra - journey

Hey guys,
I have been dealing with this condition for around five years and I can really say that I have tried everything. From topical creams for fungal acne, seb derm, steroids, to even drinking doxycycline and roaccutane and much much more. But of course nothing helped, except accutane but only periodically. Nothing addressed the oozing crusts directly so I decided to give Soolantra a try.

Every topical cream that I put on my face results in numbers of crusts all over my face that ooz yellow/gold liquid. I am 100% sure that it is going to be this way with Soolantra also but as most of you were mentioning the famous die off phase, Im mentally preparing for the worst scenario and Im willing to give Soolantra time to work.

I took two weeks off at work starting from Friday, and Im planning from that day onwards to use Soolantra daily and also document the progress here.

Soolantra was the only creme that helps me. And i use cln face wash.

All the best with it. it’s important to keep going. You only need a very small amount of Soolantra in the 5 spots on your face.

Update after two applications of Soolantra only in my beard area:

I used Soolantra only in my beard as I was afraid of a breakout because Im having a very important project going on at work. But the worst happened. The right side of my face, exactly the place where I applied Soolantra broke out with over 20 crusts, and theyre also spreading on other parts of my face and even neck. Im not sure that this is a regular die off reaction as i really have some big crusts breaking out currently and now I really dont know if I should even continue with Soolantra. The only other option is to go back to using accutane :frowning:

I wash my face first with cln face wash and after i use nivea creme. After that i use soolantra.

Luckily my vacation days started today so I am able to start my Soolantra journey.

Few moments ago I applied Soolantra on my entire face and I applied only pea-size amount for my whole face. Please if someone could help me and tell me is it really neccessary that I apply a pea-size amount per every part of my face (cheeks, forehead, beard area)?

My current situation is that I have a few minor crusts in the middle of my forehead, three crusts on the left side of my face in the beard area, and the worst break out can be seen on the right side of my face where I have crusts all over the place. So all in all the situation is very bad.

I will keep updating my journey here, as hopefully it will help someone in the future.

Praying that Soolantra helps me because life really loses its meaning when fighting with this disease.

Update after DAY 3 of applying Soolantra

I’ve practically not left the house in the past three days, but I still haven’t had a strong reaction or die-off from the cream like I did last week, when my whole face broke out just from applying Soolantra to my beard.

If I can draw a conclusion after only three days of using Soolantra, it’s that I’m still getting new crusts—but they’re significantly smaller than the ones I’ve been getting over the past 4–5 years. My beard area has become extremely dry from the Soolantra, but I won’t apply any moisturizing cream unless I absolutely have to leave the house.

The left side of my face is, let’s say, pretty clear. I only have one minor, newly formed crust and an older one that’s still healing from last week.

My forehead has always been the main problem, and it still is. I have minor crusts alongside both eyebrows and in the center of my forehead. There are also a bunch of smaller blemishes, and I wonder if that might be part of the die-off reaction.

The right side of my face is still recovering from last week’s reaction, with the occasional minor crust.

Update - DAY 5
Reaction on my forehead after 5 days of applying Soolantra. Could this be a possible die off?

I have never had such rash on my forehead as Im always getting those well known crusts filled with yellow/gold puss. These are, judging by the looks, just classic whiteheads.

Im not 100% sure should I continue to apply Soolantra also in this area where I have the rash, but I guess i will just go through it and hope for the best.

Update - DAY 9

As it can be seen on the photo, the die off reaction on my forehead still hasn’t subsided yet. In the last two days the yellow crusts are slowly coming out and hopefully i wont be getting new ones.

Other parts of my face are mostly clear outside of the right side of my face along the hairline where I have a couple of small spots which are most probably going to develop into smaller crusts.

The bottom line after 9 days is that I am STILL getting new crust every day, but they are significantly smaller than before using Soolantra. Furthermore, I think that i still haven’t passed the so called die off stage as my forehead is full of yellow crusts that formed 4,5 days ago after the start of applying Soolantra on my whole face. Definitely planning to continue the therapy and hope for the best as it has only passed 9 days.

I opened this topic exactly with the hope that someone who deals with the same problem hopefully finds this subject and encourages himself to start using Soolantra.

First of all I want to thank every and each one of the people who had at least one post on this beautiful website, it helped me a lot.

When did it help the most?

Exactly during the famous die off period when thing were awful, I was reading your posts and prepared myself that I will have to wait for improvement. And guess what? Im currently on DAY 29 since starting the Soolantra therapy and things are going magnificient. Im living my life to the fullest once again after 5,6 years of endless crying, suffering, planning my activities in the last moment because I was afraid of breaking out in any given moment, running away from people and all the other things that people with this condition are facing. I cant describe in words how grateful I am for finding this website. Thank you from the bottom of my heart. <3

Just a little bit of a chronology related to my Soolantra journey:

19.07.2025. Day 1 - no reaction

20.07.2025.Day 2 - die off phase started and it was horrific, I broke out all over my face and even my neck was breaking out also

i took a 5 day break from using Soolantra because I was waiting for my vacation days to start so I can go through the die off phase at home

Die off phase lasted from 25.07. Until 09.08. But even during that time I noticed that the crust are getting smaller and smaller, and they were breaking out more rarely than usual

From 10.08. Until 23.08. (Today) i got only one small, minor crust on the top of my forehead and one next to my left eye but it lasted only a couple of days, other than that my face is clear…itching greatly subsided also and the whole process of getting up in the morning is easier and less stressful as I m not expecting 3,4 new formed crusts to appear on my face…the bottom line is that after 29 days of using Soolantra Im seeing a great difference in all symptoms realted to demodex and I encourage everyone who is dealing with this kind of a problem (a bump that looks like a pimple but is oozing yellow liquid and forms a yellow/orange crust on top) to try Soolantra and to give it time to work!

just as a reminder I used and tried everything even Roaccutane and nothing made so much difference as Soolantra

Amazing! Sounds like you had it really bad but hopefully now it will be totally manageable. What’s the latest?

Hi there,

I can’t even express what I’m feeling right now. Soolantra has helped me tremendously, and I’m currently on day 48 of applying the cream. During this period—aside from the initial die-off reaction—I only had a couple of really, really small crusts. The bottom line is that I’ve managed to go ten days without getting anything new on my face, and in the past, that was unimaginable for me.

Additionally, before using Soolantra, I had a lot of problems with dead skin building up on my face throughout the day. Soolantra addressed that issue as well. It hasn’t cured the problem completely, but I’m no longer getting that heavy layer of dead skin that I used to have to exfoliate off every single day.

All in all, I feel free again—living my life with the same freedom I had before I got this disease. I encourage everyone to at least try and give Soolantra a shot. If your problem is Demodex-related, then Soolantra is 100% the solution
